Sweet Education Program

Our Diabetic Care Program provides skilled care by registered nurses and multidisciplinary team under the orders of the patient’s physician to provide evidence-based treatment to promote positive health outcomes as well as an improved quality of life. This is achieved through patient and caregiver education, assessment, monitoring, client education materials and collaboration with the patient’s physician with special attention given to the prevention of complications associated with diabetes.

Patients who would benefit from our Diabetic Care Program are those with frequent hospital admissions, diabetic wounds or foot ulcers, multiple new medications, patients new to insulin therapy, underlying disease processes that may contribute to poor diabetic control.

Our Program Components include:

  • Dietician Consult
  • Collaboration with the patients physician
  • Customized client education materials
  • Nutritional assessment
  • Foot assessment with follow-up education

The goal of the Diabetic Care Program is to enhance the well-being of our patients by reducing the number and frequency of hospital admissions and visits to the emergency as well as diminishing the complications and risk factors associated with diabetes.
