New Balance: Fall Prevention and Balance Program

Falls can be serious and potentially life threatening. Our fall prevention program focuses on reducing the risk of injury and falls for patients that are at risk or who have fallen in the past. Candidates for this program are:

Adults with a history of falls or at high risk of falling, anyone who may have fallen or nearly fell within the last 3 months

Those who experience fatigue, dizziness or unsteadiness

Those having difficulty transferring or walking

Taking medications that cause weakness or dizziness

Who have had a stroke

Adults who have been diagnosed with a progressive neurological disease or have arthritis, joint disease or other problems with their legs

Our Program Components Include:

  • Home safety evaluation
  • Develop a plan to improve the safety, balance, coordination, and the ambulation of our-at risk patients.
  • Medication review and management
  • Decrease in re-hospitalization
  • Care team in conjunction with physician: nurses, physical therapist, occupational therapist, and social worker
