Lung Care Program

Our Pulmonary Care Program provides skilled care by registered nurses and multidisciplinary team under the orders of the patient’s physician to provide evidence-based treatment to promote positive health outcomes as well as an improved quality of life. This is achieved through patient and caregiver education, assessment, monitoring, client education materials, and collaboration with the patient’s physician with special attention given to the prevention of complications associated with pulmonary diseases.

Patients who would benefit from our Pulmonary Program are those with frequent hospital admissions, newly diagnosed/exacerbation of pulmonary illness, knowledge deficit of disease process, pulmonary surgery recovery, nutrition deficit, activity intolerance, increased risk for infection, medication management issues, and respiratory treatment or oxygen therapy.

  • Foot assessment with follow-up education

The goal of the Pulmonary Care program is to enhance the well-being of our patients by reducing the number and frequency of hospital admissions and visits to the emergency department as well as diminishing the complications and risk factors associated with pulmonary disease.
