Heart at Home Program

Our Heart at Home Program is designed to optimize the health of the patient and decrease the likelihood of readmission to the hospital. We use a multidisciplinary approach; in conjunction with the physician a registered nurse will coordinate with Heart at Home care team, which may include a nurse, physical and/or occupational therapist, a dietitian, and a social worker

Our Program Components Include:

  • Most patients on the Heart at Home Program have the following diagnoses:
    • Hypertension (High blood pressure)
    • Coronary artery disease (signs are angina or chest pain)
    • Atrial Fibrillation (Abnormal heart beat=fast or irregular)
    • Congestive heart failure ( Heart not pumping enough blood to the heart)
    • Recently had a cardiac procedure or surgery
    • Stroke (Blood flow to part of the brain stops “Brain Attack”)
  • Identify and eliminate risk factors
  • Medication review and management
  • One-on-one time with patient to understand patients lifestyle, nutrition and exercise habits and offer suggestion or modifications to help patient manage your heart condition in conjunction with physician
  • More visit scheduled at the beginning of service to decrease re-hospitalization
  • Bedside monitoring of PT/INR’s with instant results
