Bones N’ All Program

Our Orthopedic Program is designed to optimize the health of the patient and decrease the likelihood of readmission to the hospital. We use a multidisciplinary approach; in conjunction with the physician, a registered nurse and/or physical therapist will coordinate with Bone ‘N All team, which may include a nursing, physical and/or occupational therapist, speech therapist, and a social worker.

Our Program Components Include:

  • Fall prevention evaluation/Identify and eliminate risk factors
  • Medication review and management
  • Mobility instruction
  • Strengthening and conditioning
  • Independent living skills
  • Stroke Recovery or “Brain Attack”
  • Gait and balance training
  • Individualized exercise program
  • Home safety evaluation and equipment
  • Front loading care to decrease re-hospitalization
  • Bedside monitoring of PT/INR’s
  • Most patients on this program have the following diagnoses:
    • Joint replacement
    • Surgery and amputation
    • Cardiac/Pulmonary episode
    • Decreased mobility related to general weakness
